Please download and complete the following forms before you get to camp. You may bring completed forms with you when you arrive (with the exception of the transportation form, which must be faxed). International staff, you are exempt from the transportation and tax forms.
1) Transportation Form -- this must be faxed to camp PRIOR TO June 6th, 2010. The number is (928) 778-5938.
2) Tax forms -- if you were on the staff of 2009, you do not need to fill these out UNLESS you would like to make changes to last year's withholdings.
